Revoke an authorization

You can revoke your authorization at any time by sending written notice to Rxuracy in either of these ways:

• Email revoke@rxuracy.com
• Mail Rxuracy, 400 S Kansas STE 100, Topeka, KS 66603

You only need to notify Rxuracy, not any pharmacy or data source. Revoking does not affect anything done before Rxuracy received your notice.
